Disorders of Sexual Differentiation (DSD)
Clinic
Disorders
of sexual differentiation arise from abnormalities in chromosomes, gonadal
development, or hormonal production and activity. Infants and children with
disorders of sexual differentiation, as well as their families,
need a comprehensive program that provides collaborative medical,
social and psychological care. Our DSD clinic is a unique multidisciplinary
clinic that provides care to families of children with ambiguous
genitalia. Our experts include urologists, endocrinologists,
reproductive endocrinologists, geneticists, child psychologists,
a mental health nurse practitioner, nurses and social work professionals.
The team meets with families to discuss diagnosis, management,
goals and questions, and concerns surrounding genital surgery.
The entire DSD team meets on the third Tuesday of every month
to collaborate with individual families to provide a management
plan for each child’s medical

Center for Family Focused Management of Vesicoureteral
Reflux
Vesicoureteral Reflux is the backwash of urine from the bladder
to the kidneys. Reflux in the presence of bacteria can lead to
kidney infections (pyelonephritis), which can lead to kidney
scarring. The goal of managing vesicoureteral reflux is
to prevent kidney damage. Vesicoureteral reflux can
spontaneously resolve (on its own) and at other times requires
surgical correction. Management of vesicoureteral reflux may
include daily prophylactic antibiotics to prevent kidney infections,
behavioral modification, open surgery (ureteral reimplant)
and less invasive endoscopic treatment. At UCSF we
use a family centered approach to collaboratively create a
management plan for your child. We confront or adapt
to families fears of prolonged antibiotics, procedures (VCUG,
DMSA kidney scan) and surgery. Our goal is to keep a child’s
kidneys healthy using a management plan that is respectful
of the family’s goals and concerns.
A voiding cystourethrogram (VCUG) is necessary to diagnose
vesicoureteral reflux. The test includes uretheral catheterization.
Many parents have concerns about the stress of this test on their
infant or child. Our three experienced nurse practitioners
perform the VCUG and can review
the results immediately following the exam. The nurse practitioner will work
with each parent and child to make the exam quick and easy. Parents
are encouraged to bring their child's favorite DVD to the exam
for the child to watch during the test.

Continence Clinic
The UCSF Children’s Continence
Clinic is managed by our three nurse practitioners. Its focus is to help children
with various continence issues including daytime and nighttime wetting, recurrent
urinary tract infections with or without backwash of urine to the kidneys (vesicoureteral
reflux), urinating too often or not urinating enough, and constipation and stool
problems. We work with families and children to develop healthy voiding and
stooling patterns, to assist in achieving greater continence and to prevent
urinary tract infections. Treatment options include education, behavioral modification,
timed voiding, voiding diaries, conditioning alarms and less often medications,
ultrasound, biofeedback and bladder testing (urodynamics). All families are
required to attend our Pediatric
Urology Parent (PUP) Seminar prior to making an appointment
in clinic. We have found that the seminar is the most efficient way to educate
families about their child’s condition. The course helps facilitate a successful and timely treatment plan.
Valley Care Outreach Clinic (Coming Soon)
At our soon to be open outreach clinic in Pleasanton, California
we will offer the convenience of more local care to families
of the East Bay and Central Valley. Children will be treated
for a variety of urological conditions; children who require
more complex imaging or surgery will receive these specialized
services in San Francisco.
Salinas Outreach Clinic
Every other month
members of the UCSF Pediatric Urology team visit Salinas, California
to manage a satellite clinic at the Natividad Medical Center.
Here we predominantly serve a migrant population. At our
Salinas Outreach Clinic we provide the convenience of local
coordinated care to infants and children with a
wide range of urological diagnoses. Patients who require more
complex diagnostic imaging or surgical intervention are later
seen at UCSF. However, most patients have their care managed
exclusively at our Salinas Outreach Clinic.
Spina Bifida Clinic
UCSF's Spina Bifida
Program is one of only a few programs in Northern California
to offer comprehensive medical and social support for the treatment
of infants, children and adolescents with spina bifida
and other spinal birth defects involving exposed vertebrae.
UCSF's multidisciplinary clinic provides care from medical
professionals including those in neurosurgery, urology,
orthopedics, pediatric physical medicine and rehabilitation,
pediatric development and behavior, nutrition, nursing
and social work.
From a urological standpoint
our goal is to keep a child’s
kidneys healthy and protected from damage. We
also work with families to help children be continent
(both urine and stool); this maintains and promotes self-esteem.
